The 2012-13 requirements include some changes:
The vaccine against tetanus, diphtheria, and pertussis (Tdap) will now be required for students entering grades 6, 7, 8, 9, 10 and 11 born on or after January 1, 1994. The grade 11 requirement is new this year. New entrants to secondary schools (grades 6-12) are no longer required to undergo testing for latent tuberculosis (TB) infection (TST or blood-based test).
